Designed for our uninsured patients, the Traditional Plan is only $460/year for adults (valued at $700+) and $388/year for children age 12 years and younger (valued at $600+).

Periodontal Plan

Exclusively for our uninsured periodontal patients, for only $657/year (valued at $900+) the Periodontal Plan helps maintain oral health following periodontal therapy (Scaling and Root Planing).

Membership Plan Guidelines:
Plan is not to be considered insurance and is only offered to those without dental insurance. Members cannot use other dental coverage with this plan, nor can the plan be used to supplement dental insurance.
Plan is only for services performed at Dennis Dental Care, not in a hospital or other location. Specialist referrals will not honor the discount.
Plan does not apply to costs covered by worker’s compensation, or covered under auto or medical insurance.
Orthodontic treatment is excluded.
Membership is for a 12-month period beginning on the registration date.
To receive the discount, all balances must be paid in full when services are rendered.
Membership fees are due in full at the time of registration and are non-refundable even if members do not receive services.
Dennis Dental Care reserves sole discretion to refuse treatment to any plan member or dismiss any plan member as a patient.
Dennis Dental Care Membership Plans are not dental insurance. They are non-refundable and are valid for one year from the date of sign up. These plans cannot be used for products or combined with other specials or discounts.
